Port Chester Public Schools COVID-19 Testing Consent Forms

As announced at the Board of Education Meeting on December 17th, Port Chester Public Schools will continue remote learning the week of January 4th through 8th after the holiday recess.  During that week, students and employees should schedule times to come to their home school in order to be tested for COVID for free before schools reopen. If your child is enrolled in the Open Door School Based Health Center or if you would like to enroll them (click for the consent form in English or en Español), you can make an appointment for your child to be tested by calling their SBHC directly or 914-620-9641.

If you prefer to not have your child enrolled with Open Door, then they can be tested by the school nurse at your home school. In order to do so, please complete this COVID-19 testing consent form; this form can also be found and completed by visiting our websites.  Please complete one form for each child who is attending or plans to attend in-person learning in our public schools this year.

We hope to return to in-person learning as early as is safely possible in the New Year, and this testing program is an important part of our plan to do so.  When it comes to in-person learning, we will continue with daily temperature checks, health screening forms, mask-wearing, social distancing, and other safety measures to try to provide for the safest environment for everyone.

Thank you for completing the online consent form, and please stay safe and well by following the guidance to minimize social gatherings, wear a mask, use good hand hygiene, and do your part to stop the spread of coronavirus.

Once a consent form is completed online, parents/guardians should call their child's school to schedule appointments (after January 5th) with school nurses who will be administering tests.

Como se anunció en la reunión de la Junta de Educación el 17 de diciembre, las Escuelas Públicas de Port Chester continuarán con el aprendizaje remoto durante la semana del 4 al 8 de enero después del receso de vacaciones. Durante esa semana, los estudiantes y empleados deberán agendar citas para asistir a su escuela asignada con fin de hacerse la prueba de COVID gratis antes de que las escuelas vuelvan a abrir. Si su hijo(a) está inscrito(a) en el Centro de Salud Escolar (CSE) Open Door o si desea inscribirlo(a) (haga clic para obtener el formulario de consentimiento en inglés o en español), puede hacer una cita para que su hijo(a) sea examinado llamando directamente a su CSE o al 914-620-9641. 

Si prefiere que su hijo(a) no se inscriba en Open Door, la enfermera de su escuela puede evaluarlo(a). Para que esto ocurra, complete este formulario de consentimiento para exámenes COVID-19; este formulario también se puede encontrar y completar visitando nuestros sitios web. Por favor complete un formulario por cada niño(a) que asista o planee asistir al aprendizaje en persona en nuestras escuelas públicas este año.

Esperamos volver al aprendizaje en persona tan pronto como sea posible con seguridad en el Año Nuevo, y este programa de pruebas es una parte importante de nuestro plan para hacerlo. En lo que respecta al aprendizaje en persona, continuaremos con controles diarios de temperatura, formularios de detección de salud, uso de máscaras, distanciamiento social y otras medidas de seguridad para tratar de brindar el entorno más seguro para todos.

Gracias por completar el formulario de consentimiento en línea, y manténgase sano y salvo siguiendo la guía para minimizar las reuniones sociales, use una máscara, use una buena higiene de manos y haga su parte para detener la propagación del coronavirus.

Una vez que se complete el formulario de consentimiento en línea, los padres/tutores deberán llamar a la escuela de su hijo(a) para agendar sus citas (después del 5 de enero) con las enfermeras escolares que administrarán las pruebas.

Welcome!

A National Blue Ribbon School of Excellence serving about 1,000 students in grades 6-8,  we  offer a solid educational and nurturing environment in which learning and the personal growth of each student are given the highest priority.  Our students can take advantage of a range of academic experiences including foreign language instruction, physical sciences taught in our unique Planetarium, hands-on experimental science courses in our state-of-the-art science labs, and accelerated English, math and science strands for gifted students.  Our rigorous curriculum is supplemented with living history days, school trips to theaters, historic sites and visits to cities such as Washington, DC.  We also hold an exchange program with a middle school near Madrid, Spain.

Character education plays a significant role in our school.  Our students and faculty have given much time and resources to many successful community service projects.

Finally, most of our students are active in extra-curricular activities, including band and orchestra, drama, choral groups, sports teams, yearbook, school newspaper and other clubs and groups.
